Overview
This role maintains a caseload of vulnerable older adults who need protection from abuse, exploitation, abandonment, or self-neglect. The care manager conducts investigations, completes psychosocial assessments, develops care plans, and may recommend court intervention such as guardianships or mental health commitments. Work follows federal, state, and local protective services regulations, with documentation in the Statewide Data Reporting System. The position includes on-call rotation and coordination with families, agencies, and community resources.
Requirements
- One year as an Aging Care Manager 2 or Aging Program Assessor, or three years of public or private social work experience including one year of direct care management, or an equivalent combination
- Bachelor's Degree supplemented by 12 semester hours in sociology, social welfare, psychology, gerontology, or related social sciences, or equivalent coursework
- Knowledge of protective services laws, rules, and regulations
- Computer skills for the Statewide Data Reporting System
- Ability to plan and organize work, set priorities, and maintain a caseload
- Ability to clearly express ideas orally and in writing and maintain confidentiality
