Overview

St. Clare Health Mission seeks a bilingual Hmong Community Health Worker to work primarily at the Hmong Cultural and Community Agency in partnership with Emplify Health. The role focuses on increasing patient engagement, reducing no-shows, and tracking social drivers of health. The CHW coordinates care with medical providers and social service agencies to improve outcomes for high emergency department users. Full CHW training and on the job training are provided as needed.

Requirements

• Preference of 1+ years health/social services experience and/or education
• Verifiable good driving record and reliable transportation
• Driver's License required
• Compassion, humility, empathy, and understanding of marginalized communities
• Ability to commute or relocate to La Crosse, WI 54601
• Willingness to complete CHW certification training
