Overview

WARM, part of Maitri Path to Wellness, is hiring an Outreach Coordinator for its Illinois RCCA/AHP WARM grant. The role delivers community outreach and warm handoff services, connecting people with opioid or polysubstance use disorder to overdose prevention resources, treatment, Medication Assisted Recovery, and community support. Priority is given to individuals in housing transition, exiting incarceration, or exiting inpatient treatment. This is a field-based, individual-contributor peer support and care coordination role, not a clinical position.

Requirements

• Valid Illinois driver's license and reliable personal transportation
• Ability to travel locally up to approximately 50% of work time
• Strong relationship-building, verbal, and written communication skills
• Proficiency in Microsoft Office and standard documentation systems
• Flexible schedule including occasional evenings and weekends
• Ability to lift or move up to 25 pounds and work in varied field settings and weather
