Overview
The LPN delivers shift nursing services in home and community settings for individuals with intellectual disability, developmental disability, and autism. Duties include monitoring health status, administering medications and treatments, documenting care, implementing physician orders and care plans, and supporting activities of daily living. The nurse collaborates with RNs, Program Specialists, Direct Support Professionals, and families, observes and reports changes in health or behavior, and helps maintain a safe, respectful environment aligned with PA ODP's Everyday Lives principles. The position also involves team meetings, regulatory compliance, and continuous quality improvement.
Requirements
- Current, unencumbered Licensed Practical Nurse (LPN) license in the Commonwealth of Pennsylvania
- Demonstrated clinical skills in medication administration, basic assessments, documentation, and infection control
- Experience or strong interest in working with individuals with intellectual and developmental disabilities and autism
- Ability to follow person-centered care plans and collaborate with interdisciplinary teams, individuals, and families
- Strong communication, observation, and critical-thinking skills with attention to detail and accurate recordkeeping
- CPR/First Aid certification (or willingness to obtain), valid driver's license, and ability to meet any required background checks
