Overview
The Community Support Program provides short-term, intensive case management to clients with behavioral health and substance use disorders. You will help clients reach goals, live independently, and connect to community resources. The role involves outreach in homes and community settings, documentation, and coordination with team members. Multilingual candidates are strongly preferred.
Requirements
- Bachelor's degree in psychology, social work, sociology, or related field, or minimum 3 years related experience
- Good time management, verbal and written communication, and interpersonal skills
- Personal, insured vehicle with clean driving record (driving record will be verified)
- Multilingual preferred (Spanish, Portuguese, Haitian Creole, Cape Verdean Creole)
