Overview
The Secured Living Initiative Case Manager provides housing-focused case management to individuals and households experiencing housing instability. Duties include helping participants obtain and maintain stable housing, applying for public benefits, coordinating with landlords and service partners, and documenting services in CaseWorthy. The position is on-site in Flossmoor with required participant home visits and community-based work.
Requirements
- Bachelor's degree in social work, human services, psychology, public health, sociology, or related field preferred
- Relevant housing, case management, or human-services experience may be considered
- Experience in supportive housing, housing navigation, HIV services, benefits navigation, or community-based case management preferred
- Strong case documentation, organization, and time-management skills
- Reliable transportation and ability to travel throughout South Suburban Cook County
- Experience with electronic case-management systems preferred; CaseWorthy a plus
