Overview
The Care Coordinator provides low-threshold services at the Integrity Wellness Center, a stigma-free environment for people who use drugs, sex workers, and people living with HIV/AIDS and/or Hepatitis C. Duties include screenings, individualized service plans, referrals, benefits navigation, and documentation. The role requires a high school diploma or GED and at least one year of care coordination or case management experience.
Requirements

- At least one year of care coordination or case management experience
- Commitment to non-judgmental, person-centered care
- Experience in community-based or low-threshold service settings
- Knowledge of trauma-informed care and Motivational Interviewing
- Fluency in Spanish a plus
