Overview
The Addiction Center of Broome County is hiring a Health Home Care Manager to coordinate services for a caseload of approximately 40 to 45 adult clients. The role involves administering comprehensive assessments, advocating for medical and behavioral care, and assisting clients with public benefits and Medicaid. Work is performed in person under the supervision of the Program Supervisor.
Requirements
- Associate degree with 2 years direct service experience or Bachelor degree with 1 year experience
- Experience serving individuals with mental illness, developmental disabilities, or substance abuse
- Ability to legally operate a vehicle in NY State and per insurance carrier requirements
- Familiarity with computer programs such as Word and Excel
- Ability to climb stairs, bend, stoop, sit, stand, reach, and lift up to 40 lbs
- Strong communication, organizational, and problem-solving skills
