Overview
The Health Home Enrollment Specialist completes outreach and enrollments for community members referred by internal and outside agencies. This role handles direct referrals for clients needing immediate assistance and coordinates at least 30 to 40 enrollments each month. The position operates under the supervision of the Program Supervisor and Program Director in an in-person setting.
Requirements
- High school diploma with 5 years experience, Associate degree with 2 years, Bachelor degree with 1 year, or NYS RN license with Bachelor degree
- Experience serving individuals with serious mental illness, developmental disabilities, or substance abuse
- Ability to legally operate a vehicle in NY State per insurance carrier requirements
- Ability to climb stairs, bend, stoop, sit, stand, reach, and lift up to 40 lbs
- Familiarity with computer programs such as Word and Excel
- Knowledge of eMedNY and PSYCKES to review client eligibility
